Has anyone been reimbursed from VA for their certifications from IFSEA or MHA?

As recommended by the VA Website: www.gibill.va.gov I filed a VA Form 22-1990, Application for VA Education Benefits, Application for Licensing and Certification Testing Fee Reimbursement, copies of my test scores, certification paperwork and DD Form 2366 (MGIB Eligibility), though not required I wanted to prove my eligibility.  Mailed the request, 18 Mar 05 and included a U.S. Postal Service Delivery Confirmation Receipt.  Request received by VA, 20 Mar 05.  Pretty easy so far, two days later I received, The MGIB Handbook of Educational Benefits, with no explanation of my error.  I took the class in July and got reimbursed in October, but had to send it in twice due to a problem described below.

The following information is taken directly from VA Pamphlet 22-90-2, Revised July 2001

Tests for Licenses or Certifications:  Send VA a copy of your test results with a note or a VA Form 21-4138, Statement in Support of Claim, stating that you're requesting reimbursement.

Include:
-Name of the test you took

-Name and address of the organization issuing the license or certificate (not necessarily the organization that administered the test)

International Food Service Executives Association, 304 West Liberty, Suite 201, Louisville, KY 40202-3012..

Certifications are listed by organization and also by state – IFSEA’s state with the VA is Nevada.

-Date you took the test

-Cost of the test – ($399 for the 3 tests or if you need to break it down use $195 for HACCP, $99 for CPFM, $105 for whichever you got, MCFE, CFE or CFM)

-The following statement "I authorized release of my test information to VA"

- On the envelope put – Veteran’s Affairs, Attn: Licensing and Certifications (My paperwork got held up the first time because I did not do that).

More helpful information...
Can you receive benefits for a test for a license or certification?

 -  You may receive benefits for a test you took on or after March 1, 2001 to obtain a license or certification.  You can't receive benefits for other fees relating to a license or certification. (Study Guides)  However, many courses leading to a license or certification are also approved for benefits.

-  You may take as many tests as you need.  You don't have to pass the test to receive benefits.  You can receive benefits to retake a test you failed, and to renew or update your license or certification.

-  VA will reimburse tests for licenses or certification; you receive 100% of the charges up to a maximum of $2000.00 per test.
